Make a Lost Pet Report

Please use the following form to make a lost pet report or contact us at (715) 232-9790.

  • Please complete this report to inform us of a lost pet. Most fields are required to ensure consistency. Any details help us accurately identify pets, so please be specific (especially with location lost and identifiable markings).

    Including a picture of your pet is also extremely important to accurately identify them, should they come in to our shelter. You will have a chance to upload a photo at the end of this form.

    If you have lost multiple pets, please fill out a report for each of them separately.

    Accepted file types: jpg, gif, Max. file size: 20 MB, Max. files: 2.
    • If your pet(s) does end up being found and brought here, there will be a $35 impound fee (subject to change) and we will need proof of the state-required rabies vaccination. There is also a boarding fee of $20 per night, when applicable.

      If your pet is not up to date with a rabies vaccination, we can release your pet only after seeing a receipt that you pre-paid for the rabies vaccination at a local vet clinic.

      If you live in the City of Menomonie, a pet licenses is also required, but we have those available for purchase if you don't already have one.

      Requirements for release and associated fees are non-negotiable.

      Feel free to contact local law enforcement to let them know you are missing your pet in case they are called to pick up a stray pet in your area. Please call the non-emergency numbers:

      - Menomonie Police Departement (715) 232-1283

      - Dunn County Sheriff Department (715) 232-1348

      - University of WI-Stout Police (715) 232-2222 (on campus dial 2222)
